Using the Clinical Notes tab
The Clinical Notes tab is the default view when you open Templates. It's where you manage all templates used for generating in-session notes. V2 templates give you drag-and-drop control over structure, conditional logic, rules, and AI content blocks.
Creating a Clinical Notes template
Click Create Template — this defaults to V2. You have three options to get started:
Import from Notes — paste an existing clinical note and PatientNotes builds a structured template from it automatically.
SOAP template — start with a pre-built Subjective, Objective, Assessment, Plan layout.
Start from scratch — build section by section using the Custom tab.
If you prefer to keep using V1, you can still create a legacy template from the same dropdown.
Drag and drop editor
Reorder sections by dragging them, rename headers, and add spacing for better formatting. The main formatting menu controls header style (Normal text, H2, H3), alignment, bold, italic, and includes undo/redo.
Content blocks
Use the Custom tab to add individual blocks to any section:
AI Text — narrative paragraph output
AI List — bullet or numbered list output
AI Tables — structured table output (new in V2)
You can also add merge fields and custom context areas within any block.
Rules — control your output format
Every Clinical Notes template has a Rules tab. Click on any section to choose its output format: sentence, paragraph, direct quote, list, table, or labelled sections. Additional rules from Add Rules include punctuation, level of detail, information order, time and date formatting, and measurements.
Conditional logic
Click on any section and tap the green Make Conditional button to set it to only appear when the relevant topic was discussed. You can also type a specific condition to define exactly what triggers the section. To remove it, double-click. Empty sections will never appear in your notes.
Craft with AI
Open the Craft with AI tab to improve any section by giving plain-language feedback. You can also upload an existing note or V1 template and Craft with AI will convert it to V2 format automatically.
Testing your template
Use the Test tab to run your template against a real client session before going live. If something needs adjusting, return to the Structure tab and either give feedback via Craft with AI or edit manually in the Custom section.
Configuring template order
Click the Configure button to drag and reorder your templates. The order here controls what appears in the dropdown when you start a new session.
