What's new in Templates V2
We've listened to your feedback and made significant changes to templates. V2 templates are now available across Clinical Notes, Letters, Reports, and Patient Summaries — giving you far greater control over your output through a new rules system, conditional logic, and an AI agent called Craft with AI.
This walkthrough shows you how to enable and use Templates V2 - the unified editor for clinical notes, letters, reports, and patient summaries.

New navigation — four tabs
When you open Templates, you'll now see a tab bar with four sections:
Clinical Notes — in-session note generation (default tab)
Letters — referral letters and clinical correspondence
Reports — formal reports including medico-legal and progress reports
Patient Summary — structured patient summaries
Your existing prompts and V1 templates still work
Nothing has been removed. You can continue using your existing prompts or V1 templates exactly as they are, or choose to upgrade them to V2 templates at any time. When creating a new template, you'll see the option to create a V2 template (default) or a legacy one — both are supported.
Drag and drop editor
The V2 editor gives you full control over your template structure. You can drag and drop sections to reorder them, rename headers, add spacing for better formatting, and control the specificity of language and tone for both personal and organisation templates.
Content blocks — AI Text, AI List, AI Tables
Using the Custom tab, you can build templates from individual content blocks:
AI Text — narrative paragraph output
AI List — bullet or numbered list output
AI Tables — structured table output (new in V2)
You can also add merge fields and custom context areas within any block.
Conditional logic — If Discussed, Then produce
V2 introduces logic blocks. Click on any section and you'll see a green Make Conditional button. Apply it and that section will only appear if the relevant topic was discussed in the session. You can also type a specific condition to define exactly what triggers the section. To remove it, double-click. Empty sections will never appear in your notes.
Rules — format control in a simple interface
Every template has a Rules tab. Click on any section and choose your output format: sentence, paragraph, direct quote, list, table, or labelled sections. Additional rules available from Add Rules include punctuation, level of detail, information order, time and date formatting, and measurements.
Header formatting is controlled from the main formatting menu — Normal text, H2, H3, alignment, bold, italic, and undo/redo.
Document Rules for Letters and Reports
Letters and Reports now have their own Document Rules — covering tone, length, formality, and language style. If you upload an example letter or report, the system will do its best to match that style automatically.
Craft with AI — just give feedback
Craft with AI is an AI agent built into the template editor. Describe what you want changed in plain language and it will update the template for you. You can also upload an existing clinical note or template and it will convert it to V2 format automatically.
Test before you go live
Use the Test tab to run your template against a real client session. If something needs adjusting, return to the Structure tab and either give feedback via Craft with AI or make manual adjustments in the Custom section.
Getting started
Go to patientnotes.app/dashboard/templates. If you don't see the new interface, go to Settings → Profile → Early Access Features and enable Extended Templates.
