If you have existing prompts you're happy with, you can upgrade them to the new template editor using the Upgrade Template button. This converts your prompt into a structured template with content blocks and format rules.
Important: this action is not reversible. Once a prompt is upgraded to a template, it cannot be converted back. PatientNotes will offer to keep a copy of the original automatically — we recommend leaving this on.
How to upgrade
Find the prompt you want to upgrade and click Upgrade Template
A confirmation modal will appear:
The Keep a copy of the original checkbox is ticked by default — leave this on
Click Upgrade to proceed
If the checkbox was ticked, a duplicate of your original prompt will appear in the list with — original added to the name
The upgraded template opens in the new editor ready to review
Keep a copy — why it matters
The Keep a copy of the original option is checked by default for good reason. The upgrade creates a strong starting point but may not perfectly replicate every nuance of your original prompt. Having the original means you can:
Compare the output side by side
Refer back to the original wording when refining sections
Fall back to the old prompt if needed while you refine the upgraded version
Once you're happy with the upgraded template, you can delete the — original copy at any time.
After the upgrade — review and refine
The upgrade creates a starting point — not a finished template. Check that:
All your sections have been detected and named correctly
Content blocks are set to the right output style for each section
Format rules reflect your preferred tone, tense, and detail level
Then go to the Test tab and run it against a real or sample session. Adjust anything that doesn't look right, then test again until you're happy with the output.
What about System Prompts?
System prompts (labelled System Prompt in your list) cannot be upgraded — you can view them but not edit or convert them. Use them as a reference when building your own template from scratch.
