Stop "Not assessed", "N/A" or empty sections appearing in your notes

Stop "Not assessed", "N/A" or empty sections appearing in your notes: make the section conditional in V2 templates, refine a single note, or use If Discussed in V1 templates.

Seeing sections in your note that say "Not assessed", "N/A", "Not discussed", or headings with nothing underneath? Here's how to stop them appearing.

V2 templates: make the section conditional

In V2 templates, sections appear every time unless you make them conditional. A conditional section only appears when that topic actually came up in the session.

  1. Open your template from Templates.

  2. Click the section that keeps appearing empty.

  3. Click the green Make Conditional button. You can also type a specific condition for when it should appear.

  4. Save, then use the Test tab to check it against a real session.

Or open Craft with AI and ask in plain language, for example: "Only show the Medications section if medications were discussed, and never write 'not assessed'." How to use Craft with AI

Need every section to show every time? Some clinics require a complete record, including items that weren't assessed. In that case, leave the section unconditional so it always appears.

Fix it for just one note

Use the refine / feedback box on the Clinical Note tab, for example: "Remove any section that says not assessed or not discussed." Refining a note

Using a V1 (legacy) template?

V1 templates use an Always / If Discussed setting instead. Set the heading and its content to If Discussed. V1: Always vs If Discussed

If you're still using an old-style prompt, you can add this instruction to it:

"Use only the transcript, contextual notes or clinical note as references. Never add your own details about the patient, assessment, diagnosis, interventions, evaluation or plan. If the information for a section is not explicitly mentioned, leave that section out instead of noting its absence."

Better still, convert it to a V2 template — Craft with AI can do this for you automatically.
