Since moving to the new PatientNotes, are your notes needing more editing than before? Common signs:
Notes need far more editing than they used to
You have to reapply or reselect your template
Formatting you didn't ask for, such as bold text or extra headings
Findings from your examination written as if the patient reported them
The note only shows your template's headings and wording, with no content
In most cases the fix is the same: move your template to a V2 template. Older prompts and V1 templates still work, but V2 templates give you much more control over exactly how the note is written.
Step 1: Upgrade your template to V2 (about 5 minutes)
Go to Templates. If you don't see the new template editor, go to Settings → Profile → Early Access Features and turn on Extended Templates.
Create a new template (V2 is the default), open Craft with AI, and paste in your old prompt or an example of a finished note you like (de-identified). Craft with AI converts it to a V2 template for you. How to use Craft with AI
Run it in the Test tab against a real session, adjust, and save.
Select it from the template dropdown when you start your next session.

Step 2: Fix specific problems
Examination findings written as if the patient reported them
Tell the template exactly what each section should contain. In Craft with AI, try: "The subjective section should only include what the patient reported. Put my examination and assessment findings in the objective section as my clinical findings, never as patient-reported."
Unwanted bold, headings or formatting
Open the Rules tab and click the section to set its output format (sentence, paragraph, list, table or labelled sections). Or describe it in Craft with AI, for example "Plain text only — no bold."
Notes too long or too detailed
In the Rules tab, use Add Rules to lower the level of detail. Template Rules guide
Only headings, no content
The sections are set as fixed text. Add an AI Text, AI List or AI Table block under each heading with a short instruction. Step-by-step
"Not assessed" or empty sections
Make the section conditional so it only appears when discussed. How to stop empty sections
Just need to fix one note?
Use the refine / feedback box on the Clinical Note tab to ask for the change in plain language. Refining a note — then update your template so you don't have to do it every time.
Still not right?
Chat with us via the help widget or email [email protected]. Send a de-identified example of the note you got and how you'd like it to look, and we'll fix the template with you.
